Telehealth Informed Consent

Please read this consent form carefully before using telehealth services coordinated through Healthville Pharmacy.
By proceeding with telehealth services, you acknowledge that you have read, understood, and agree to the terms below.
1. What Is Telehealth?
Telehealth refers to the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. This may include video consultations, telephone calls, secure messaging, or the electronic transmission of medical information for the purposes of diagnosis, treatment, or consultation.
2. Role of Healthville Pharmacy
Healthville Pharmacy coordinates telehealth services between patients and independently licensed healthcare providers. We are a compounding pharmacy; our pharmacists do not diagnose conditions or prescribe medications. The prescribing authority rests solely with the licensed practitioners you are connected with through our platform. We fulfill valid prescriptions issued by those practitioners.
3. Benefits of Telehealth
- Convenient access to licensed healthcare providers without in-person travel.
- Faster access to consultations and prescription management.
- Continuity of care for established patients.
- Access to specialty compounded medication services.
4. Risks and Limitations
Telehealth services involve certain risks and limitations, including but not limited to:
- Information transmitted may be insufficient for complete clinical evaluation (e.g., no physical examination).
- Technical failures may interrupt or prevent a telehealth session.
- Security protocols may not protect against all unauthorized interception of confidential information.
- Telehealth may not be appropriate for all medical conditions. Your provider may determine that an in-person visit is necessary.
5. Your Rights
- You have the right to withhold or withdraw consent at any time without affecting your right to future care.
- You have the right to ask questions about the telehealth process at any time.
- You have the right to a copy of any health records generated during your telehealth encounter upon request.
- You have the right to seek care from an in-person provider at your discretion.
6. Confidentiality
All telehealth communications are subject to the same confidentiality protections as in-person visits, including compliance with the Health Insurance Portability and Accountability Act (HIPAA). We use encrypted, HIPAA-compliant technology to protect your health information. However, as with any electronic communication, complete security cannot be guaranteed. 7. Technical Requirements
To participate in telehealth services, you are responsible for ensuring that you have:
- A reliable internet connection.
- A device with a working camera and microphone (for video consultations).
- A private location where you can speak freely.
8. Emergency Situations
Telehealth is not appropriate for medical emergencies. If you are experiencing a life-threatening situation, call 911 or go to your nearest emergency room immediately.
