Prescriber-authorized refills only

A refill request is not a new prescription. Healthville Pharmacy will dispense a refill only after our pharmacy team verifies that authorized refills remain on file for this medication. If no authorized refills are on file, we will contact your prescriber to request authorization. This form does not authorize dispensing of any controlled substance, new medication, or unauthorized refill.
